Study on the Clinical Utility of "Emotional Blunting Scale


Abstract
Since Bleuler, "emotional blunting" has been considered a core symptom in schizophrenia, but most of the diagnostic criteria except "Present State Examination of Wing" excluded "emotional blunting scale" because of its allerged unreliability. So Abrams and Taylor presented "Rating Scale for Emotional Blunting" in 1978. Using this scale, the authors studied on the inter-rater reliability of the "Emotional Blunting Scale" and studied also that this scale can discriminate between schizophrenia and major affective disorder.
The results are summarized as follows
1) At the time of admission, using "weighted kappa of Cohen" for each item, the inter-rater reliability proved to be high except item 10 (lacks spontaneity) and item 12 (indifferent to surroundings).
2) At the time of discharge, also using "weighted kappa of Cohen" for each item, the inter-rater reliability turned out to be high except item 9 (difficult to excite emotions/ unresponsive) item 12 (indifferent to surroundings) item 13 (indifferent/lack of affecton for family, friend) and item 14 (indifferent/unconcern for own present situation).
3) Using "Spearman¢¥s rank correlation coefficient" for the total score of the two raters, the degree of association was also highly significant both ¢¥at the time of admission and discharge.
4) The mean "Emotional Blunting Scores" of schizophrenia were significantly higher than those of major affective disorders. This means higher "Blunting Scores" would be associated with diagnosis of schizophrenia and the lower ones with diagnosis of affective disorders.
